POSITION DESCRIPTION:
This position will be assigned to the Regional Office Region 4 Front Door Team. As such will assist individuals and families seeking and/or amending OPWDD services from all 5 boroughs.
Primary Duties and Responsibilities
Staff assigned to the Front Door division at the Regional Offices serve in a variety of capacities including, but not limited to:
Front Door Facilitator (FDF) o Will serve as the liaison to the families seeking to obtain services at first point of contact. o Will assist with conducting information sessions, follow up on eligibility status, conduct assessments, complete pre-authorizations, consult with Care Coordination Organizations, and complete and mail authorization forms. o Will address all issues that may arise as families begin and transition throughout all the various processes involved in establishing eligibility and obtaining services. o Will interface with hundreds of newly identified families per month through required sessions that introduce them to OPWDD services. o Will continue oversight to the families as they progress through the authorization process. o Will continue to provide ongoing assistance to the families who have not yet completed the Front Door process.

Salary Range:
From $57984 to $73813 Annually
Minimum Qualification
Permanent Service as a Social Work Assistant 3 OR Reachable on the current Social Work Assistant 3 Civil Service list OR Eligible for 70.1 transfer to a Social Work Assistant 3 title OR Provisional Appointment:
Must possess a Bachelors degree or higher in a human service area and one year of permanent competitive service as a Social Work Assistant 2 or parenthetic; For the purpose of these examinations, degrees in the following human service areas are considered qualifying:
the professional disciplines of art therapy, audiology, dance therapy, drama therapy, music therapy, nursing, nutrition, occupational therapy, physical therapy, psychology, social work, speech/language pathology, and therapeutic recreation. Also included are related academic disciplines associated with the study of:
human behavior (e.g., gerontology, human services, sociology, speech communication); human skills development (e.g., counseling, education, human development); or any other study of services related to basic human care needs (e.g., community mental health, rehabilitation counseling). Examples of non-qualifying degrees include but are not limited to:
anthropology, archeology, communication, criminal justice, fine arts, philosophy, and political science. VACCINATION REQUIREMENTS:
Must be fully vaccinated for Covid-19 based on the Center for Medicaid Services regulationEstimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
