Responsibilities:
o Under the supervision of the Treatment Team Leader and Medical Doctor, the incumbent will function as a Treatment Coordinator in the Article 16 Clinic in accordance with OPWDD 679 regulations with duties to include facilitating the intake process for individuals new to the Article 16 Clinic or for individuals already served who require additional services. o The incumbent will serve as a primary point of contact and resource between the individual, their circle of support (including residential team members, family members and care manager) and clinic providers for information about additional services, current treatment. o The incumbent will ensure Coordination of services without duplication by outside providers. o The incumbent may complete psychological or Social Work assessment as needed. o The incumbent may refer the individual for additional services based on the feedback of the clinic providers or the request of the individual served and/or their family or other circle of support members. o Perform various assessments (risk, need, adaptive and functional) and review prior evaluations and clinical plans of care. o Assist in the completion of consultation reports to include formal recommendations, referrals, treatment alternatives and service outcomes. o Conduct screenings to assist in the evaluation of service needs. o Perform clinical program, service access and quality reviews to ensure appropriateness of treatment/service provision. o Other duties as assigned to meet the needs of the bureau or division and support the goals of the agency.

Salary Range:
From $67897 to $86140 Annually
Minimum Qualification
Licensed Master Social Worker 2 - Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; OR Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying. Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying. Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
