The Waiver Unit is a key component to the core mission of our agency and is the hallmark of our transformation agenda. As such this candidate's primary function, although related to the Service Access Team, will be the primary contact for individuals transitioning from their local school district into adult services with OPWDD. Familiarity with Tracking and Billing System, CHOICES and ability to maintain databases and monitoring systems preferred. This employee will be responsible for the following:
o Complete EAAs and DDP2s based on conversation with family/individual transitioning out of their school district into services with OPWDD o Provide Front Door Information Sessions on site, communities and in schools. o Provide trainings, information and outreach to school districts. o Work with district personnel in identifying upcoming graduates, schedule Front Door Sessions and assist with Service acquisition o Track caseload using Front Door Spreadsheet and compile reports of transitioning students to interested parties. o Attend meetings with school district personnel, voluntary providers, advocacy organizations and other State agencies o Troubleshoot problems. o Complete special assignments as needed. Salary Range:
From $67897 to $86140 Annually
Minimum Qualification
Candidates must be licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; OR Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying. Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying. Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
