Responsibilities:
o Collaborate with behavioral health & crisis services to coordinate of care for individuals transitioning out of highly specialized programs into OPWDD services. o Develop local and statewide resource directories and technical guides. o Serve as the liaison between providers serving people with complex and challenging support needs, responsible for coordinating needed services for these individuals and/or their families. o Assist in the design and implementation of community-based treatment supports/programs focusing on relapse prevention, impulse and anger management, harm reduction and skill acquisition for increased self-management. o Support individual(s) in crisis and the agency/program(s) supporting them, or to prevent imminent crisis and the agency/program(s) supporting them, or to prevent imminent crisis by provision of direct case consultations, professional mentorship, supervision, training, and program monitoring. o Perform various assessments (risk, need, adaptive and functional) and review prior evaluations and clinical plans of care. o Assist in the completion of consultation reports to include formal recommendations, referrals, treatment alternatives and service outcomes. o Conduct screenings to assist in the evaluation of service needs. o Perform clinical program, service access and quality reviews to ensure appropriateness of treatment/service provision. o Member of Community of Informed Practice at the state and local level-to foster collaboration and build bridge's to community-based services, and participate in other committees/agency work-groups as delegated. o Travel as needed to support bureau operations. o Other duties as assigned by the bureau Director. o Other duties as assigned to meet the needs of the bureau or division and support the goals of the agency. o Under the supervision of the Treatment Team Leader will provide Social Work Services in accordance with the 483 and 624 regulations. o Will work as part of an interdisciplinary team to accurately identify the comprehensive needs of an individual, design, implement and document appropriate services. o The incumbent will complete the psychological and the Social Work assessment section of the individualized comprehensive functional assessment (CFA), Client Profile, Social section of the CFA which includes sexual consents, personal expenditure plan and any other required assessments in accordance with the 483 (ICF) regulations to address the individual needs, limitations and strengths. Will also be responsible for updating Psychosocial for those individuals in the community who may need one. The incumbent will maintain individuals DVP records and document all meetings and act as case manager. o Serve as a resource to other team members and to assist the CATH individuals and their families in finding the appropriate placements that best fit their needs. o Social Worker for individuals represented by CAB (Consumer Advisory Board). Will not only advocate for the individual but for their correspondence as well. o Provide individual, group as well crisis intervention. o Record significant psychosocial events and/or changes I clinical condition. o Educate individual and family about the nature of the disorder/illness and available resources and how to access them. o May direct and oversee the therapeutic interventions of their assigned individuals. o Ensure the implementation of BFDDSOO policies and practices and 483 ICF and 624 Regulations. o Provide in-service training to Direct Support Professionals in the implementation of the individualized treatment plan.

Salary Range:
From $67897 to $86140 Annually
Minimum Qualification
Licensed Master Social Worker 2 - Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; OR Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying. Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying. Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
