Habilitation Specialist 1

Under the supervision of the Home Care Liaison Supervisor or Treatment Team Leader, the incumbent will act as the Family Care Home Liaison and the duties will include but will not be limited to:
o Ensuring that the environmental requirements of the Family Care Program are being met and maintained in each Family Care Residence o Ensure that the fiscal and other Family Care Provider needs and concerns are being met, consistent with the needs of the individuals residing in the home and Federal and State Regulations o Act as the direct link to the Provider on behalf of the individuals living in family care, as well as other interested parties and providers of service o Will have a caseload of up to twenty (20) family care homes and up to 30 individuals and will be required to visit each home at least monthly. Visits may be early mornings or late afternoons o Will participate in the initial certification, recertification and special visits of each home as may be required o Will be responsible for the development of Residential Habilitation plans, Health and Safety needs, Personal Expenditure plans and Money Management assessments for all individuals residing in their assigned Family Care homes o Ensures that the FCF provider is carrying out the goals as written o Ensures that the service is recorded in TABS for correct billing o Ensure that each home has a current fire evacuation plan that meets the needs of the individuals in the home o Monitors the use of personal allowance and clothing allowance of the individuals in their homes o Attends annual and semi-annual Life Plan meetings for the individuals living in their assigned homes o May also perform in home and classroom trainings with FC providers and certified sitters as required o Should be familiar with Part 624 and Part 633 regulations. o May perform other duties as needed at the direction of the TTL o Must be current on all mandatory trainings

Salary Range:
From $45455 to $58028 Annually
Minimum Qualification
Reachable on the Civil Service List for Habilitation Specialist 1 OR Permanent service as a Habilitation Specialist 1 OR Eligible to 70.1 transfer to a Habilitation Specialist 1 title OR Eligible for provisional appointment pending examination. To be eligible for provisional appointment, candidates must possess a bachelor's degree or higher in a human services field and have one year of permanent competitive or 55-b/55-c service in a clinical or direct care position OR one year of direct care or clinical experience providing services to individuals with developmental disabilities. If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained . MUST BE CURRENT ON ALL TRAININGSEstimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
